I, hereby grant permission to KY T2 to reproduce any portion of the photo images listed below that have been taken by me or our team for their 2026 Calendar. By submitting the photograph(s), I authorize KY T2 to publish, reproduce, exhibit, transmit, broadcast, televise, digitize, display, distribute and otherwise use for educational, promotional, or any other purposes: (a) my image, and likeness (b) all photographs of or by me or my team (collectively, the Materials), in any format whatsoever now or hereinafter created by KY T2 without further notice or consent. It is understood that all of the Materials, and all reproductions, and electronic and digital copies of the Materials are the sole property of KY T2. Furthermore, I grant creative permission to alter the photograph(s). By participating, I release KY T2 and its agents from any liability arising out of use of the Materials. I understand the photographs(s) may also be uploaded to the KY T2 website, YouTube, social media (Facebook, Twitter, Instagram and other social media accounts), as well as print marketing materials, educational materials, and other material without compensation to me. I represent that I have read and understand the foregoing statement and am competent to execute this agreement.
